You must be athletically cleared for the 2026-2027 school year in order to participate in Brahma Athletics after June 1st, 2026.

In order to get cleared, you must complete the following process.

1) Receive a physical from your family physician. If you complete your physical at Kaiser, we can accept their sports physical form. Otherwise, please print the form below and bring it with you to your doctor’s appointment for them to complete.

2) Download the following forms and fill in every blank, signature, date, and initial line. Any incomplete or improperly signed forms will be rejected and you will need to resubmit. We do not accept forms with electronic signatures or initials.

3) Once you have completed your paperwork, please visit athleticclearance.com or homecampus.com to begin the online registration process. Directions can be found HERE.

4) Under file upload, please upload a copy of your physical, the district waiver, and a picture of your medical insurance card. The name on the insurance card must match the athlete. Upload multi-page files as PDFs (not .jpg or .png).

Processing times generally take two weeks from the date of submission. You must submit paperwork by the required deadlines to participate. Late paperwork will not be accepted. It is your responsibility to complete this on time.
