9:00 am - 12:00 pm
Diamond Bar High School Theater

Diamond Bar, CA 91765
Available Vaccinations: Pfizer (12 years old+) or Johnson & Johnson (18 years old +)
TO MAKE AN APPOINTMENT:
1. Go to https://myturn.ca.gov
2. Complete preliminary information and select “Continue”
3. Enter “Diamond Bar High School” into address field
WHAT TO BRING:
1. Proof of age (driver license, birth certificate, passport)
2. Proof of insurance (*if you have insurance)
If you are able bring a copy of your proof of age and insurance, it will save valuable time for you and for the clinic.
