Hello Walnut Valley Employees,

We are pleased to announce the first 2022 COVID-19 vaccination clinic opportunity for Walnut Valley staff, families, and community members.  The next clinic will be held in the Diamond Bar High School gymnasium on Saturday, January 8 from 9:00 a.m. to 12:00 p.m. 

This clinic will be for anyone who is eligible for the booster, and anyone who needs to get their first and second dose of the vaccine.

WHAT TO BRING:

1. A copy of your proof of age (ex. Birth certificate, passport, AIRES cover page for students) – we will need to keep the copy
2. Proof of insurance if applicable – we will need to keep a copy
3. Minors must be accompanied by a parent/guardian

If you are able to provide us with a copy of your proof of age and insurance it will save valuable time for you and for the clinic.
