Hello Walnut Valley Employees,

We are pleased to announce a COVID-19 vaccination clinic opportunity for Walnut Valley staff, families, and community members.  The next clinic will be held on Saturday, March 12, from 9:00 a.m. to 12:00 p.m. in the Diamond Bar High theatre.

This clinic will be for anyone who is eligible for the booster, and anyone who needs to get their first and second dose of the vaccine.

WHAT TO BRING:

1. A copy of your proof of age (ex. Birth certificate, passport, AIRES cover page for students) – we will need to keep the copy
2. Proof of insurance if applicable – we will need to keep a copy
3. Minors must be accompanied by a parent/guardian

If you are able to provide us with a copy of your proof of age and insurance it will save valuable time for you and for the clinic.
