We are pleased to offer another COVID-19 vaccination clinic opportunity for Walnut Valley staff, families, and community members.  The next clinic will be held in the Diamond Bar High School theater on Saturday, November 13 from 9:00 a.m. to 12:00 p.m. We have partnered with Mercy Pharmacy to offer the Pfizer vaccine to anyone 5 and over, the Moderna vaccine to anyone 18 and over, and the Johnson and Johnson vaccine to anyone 18 and over.  

In addition, the Pfizer, Moderna, and Johnson & Johnson BOOSTERS are now available for all Walnut Valley staff, as well as anyone over the age of 65 or anyone 18 - 64 with a certain chronic condition.

WHAT TO BRING:

1. A copy of your proof of age (ex. Birth certificate, passport, AIRES cover page for students) – we will need to keep the copy
2. Proof of insurance if applicable – we will need to keep a copy
3. Minors must be accompanied by a parent/guardian

If you are able to provide us with a copy of your proof of age and insurance it will save valuable time for you and for the clinic.
