TO MAKE AN APPOINTMENT:

5. If you have trouble with the online portal, WALK-IN’s are welcome
WHAT TO BRING:
1. Proof of age (driver license, birth certificate, passport)
2. Proof of insurance (*if you have insurance)
If you are able bring a copy of your proof of age and insurance, it will save valuable time for you and for the clinic.
